Please see the letter attached from the immunisation team regarding the 2nd dose of the Covid vaccination or the 1st if it was missed in the Autumn term.
The letter gives you all the information needed but most importantly the link and code you require to access and complete the electronic consent form. Parents/Carers only need to submit 1 form per child. If you make an error when completing a form please contact us to correct the error, rather than submit another form.

The completion date for all consent forms is 10am on Monday 10 January.
Please ensure you submit the completed form in time for the session which are being held on Wednesday 12 – Thursday 13 January 2022
